Coming home from the hospital can be difficult and stressful, our carers can help you make the transition as smooth as possible by offering appropriate and practical support as you gain strength and have more time to focus on recovery, minimising the risk of injury and re-admission.

To help patients stay at home safely, we'll collaborate with the hospital discharge team and the client's healthcare team. The service will be tailored to the client's needs.


Visit in Hospital to Assess Needs 

We will visit you at the hospital to analyse all of your needs, do a Home Risk Assessment, and then prepare everything for your return to home.


Home Risk Assessment 

A risk assessment is a method of identifying potential hazards and determining what might happen if they occur. We will inspect your home and inform you of any hazards we discover, as well as how to eliminate them for your safety.


Hospital Discharge

You go through a process called hospital discharge when you leave a hospital after treatment. 

We will work together with the medical staff to accompany and support you throughout all your hospital discharge process.


Postoperative Care

Any care you receive post a surgical procedure is described as postoperative care. The sort of postoperative care you need is based on the type of surgery you had as well as your clinical history. This usually starts in the hospital and then continues after you've been discharged and returned home. Pain management and wound care are frequently included in postoperative treatment and we will be with you to support you during all your recovery time.


Short Term Illness / Recovery

We will work together with your GP, Public Health Nurse, and other members of the Multidisciplinary Team as needed to assist and support you during your recovery.
